AW Parents,
We are very excited to announce that the Anthony Wayne Local Schools Athletic Department has partnered with FinalForms, an online forms and data management service. FinalForms allows you to complete and sign all athletic participation forms for your student(s). The most exciting news is that FinalForms saves data from season-to- season and year-to-year, meaning that you will never need to enter the same information twice! FinalForms also pre-populates information wherever possible, for each of your students, saving you time.
Once your child is initially registered, you will only be required to review their information once per school year to verify information is current and sign the forms.
Review the Parent Playbook (attached) to get started in FinalForms. If you require any support during the process, scroll to the page bottom and click “Use Support”.
We are asking that ALL parents of athletes use FinalForms. Please register at -- https://anthonywayne-oh.finalforms.com/ -- now and electronically complete ALL forms! This will be required for all AW sports beginning with the Fall 2017 season.

As of this writing, we are excited to expect indoor and outdoor athletic facilities at full capacity for Fall of 2021! As we prepare for these events, it is essential to note to spectators that ALL AWHS and AWJH home athletic events will require tickets to be purchased online. We will sell NO tickets at the ticket entrance. As of this writing, no passcodes will be necessary to purchase tickets to any athletic event, and all are welcome to purchase event tickets.

$325 (good JH and HS events)
Student All Sports Pass
$125 (good for 1 student entry JH and HS events)
Adult All Sports Pass
$150 (good for 1 adult entry JH and HS events)
JH Adult All Sports Pass
$75 (good for 1 adult entry at JH events)
